The three stars thing is the end point, although you can already move onto the next hospital with just a single star. Even after three, you can keep playing like it were a sandbox, but you gain no more rewards from the game, other than a little kudosh from regular VIP visits and yearly awards. Every star is basically just a set of increasingly harder goals for your hospital. Until the goals run out. If you then go to the next hospital, you can still go back to the old hospital from world view, to either continue it or restart just that one. Compare that to Theme Hospital, were you had just a single set of goals and could also just keep playing until you were tired of the level and move on permanently. EDIT: I aced the first two hospitals with three stars. The last star on the second hospital is a bitch. Requires you to get hospital attractiveness up to 65%. I covered all my walls in gold plaques.
