I always start my hospital without any of the specialist treatment rooms, two GP offices and all diagnosis rooms. Unless there's a particular disease that's very common in a region, Pharmacy and Psychiatry are enough to bring in money. If I manage to get overwhelmed with patients in the beginning, before everything is ready for them, I raise prices to suppress visitor numbers. I then have two training rooms on full blast. I have slightly more doctors then I need, working towards future expansion. Hopefully all my hired beginner staff is untrained, so I can specialize them myself. Taking one or two people out of rotation isn’t a problem. One of them gets a specialist lecture, then that person teaches the rest of the staff for free. My beginnings are always a bit slower, with maybe 6 doctors, 5 nurses and a handful of other staff in total, but they’re not costly in wages if they’re new and I can ramp up their number and expertise slowly with the visitor numbers. You’ll be making most of your money from diagnosis in the beginning, which suits a beginner staff. Since they're new, they'll often fail to diagnose a patient on their first try, send them for further diagnosis and that patient gets bounced around, paying money for every visit. It doesn't kill them, it's easy xp for your staff and by the end you can just send the unique cases home afterwards. It's hard not to make at least a small profit since wages aren't high. Once your staff gets better and you slowly expand, cures are your biggest money makers. By the end, my hospitals usually have 3-6 specialized GP doctors, JUST to handle the 3-5 GP offices. Since those rooms are so important, that staff is trained first and they start managing to diagnose patients on their first try. This clears up queues and makes cures more important. Then come my 5 specialized psychiatrists working my 3 psychiatrist wards, my treatment nurses, doctors, ward nurses and diagnosis nurses. Research and radiology staff come later. ![]() Treatment staff gains experience by working in diagnosis in the beginning, but by using that experience to give them treatment talents. I only build specialized treatments rooms like the de-lux, de-pan or de-clowning clinic, and assign them to it, once they've earned 2-3 treatment talents so that they don't kill people.
