Somehow I appeared to get logged out - without deleting any cookies or clicking the 'Log Out' button.

Nope.

I have had this happen to me before though. The possibilities are:
1. Someone else is logging in to your account
2. You've somehow ended up with two copies of the eg cookies in your browser

The latter one happened to me a while back. I deleted my cookies and logged back in and that's fixed it since.
